Empowering Patients Using SALT

Hello ALL,



I've been away from the site for a bit so im now back to catch up and to share a few pics from my visit to the HIV ward of Georgetown Public Hospital. Yesterday was my birth anniversary so i felt the urge to take breakfast for the patients and nurses of the HIV ward of the Hospital on Saturday.



All went well as planned, I prayed with them, we talked, laughed and i took the opportunity to empower them on being more proactive upon returning to society.

Good Communication

Clinicians are part of your clinical team. If you are in a peripheral facility, methods need to be developed for good communication and at least yearly meetings.

Communicate with the district hospital/clinic concerning all chronic patients,
even when treatment is initiated at the first level facility.

Coordinate care with appropriate clinic/clinicians.

Refer patients back to clinicians as appropriate.

I found it so unfortunate that I couldn't take your video that last day and then it turned out that the camera disappeared from the hotel! I hope we find a way to capture your story sometime in the future.

Our visit to St Francis Community Developers was an awesome opportunity. It was great to see all our dreams which was shared regarding SALT, already laid down. I'm fortunate to be a part of the SALT team and truly happy to see our vision being fulfilled in St. Francis Community. We need to work on further developing our vision along with other Organisations to empower the SALT and have it maximized throughout our beautiful country.
