Empowering Patients Using SALT

Hello ALL,



I've been away from the site for a bit so im now back to catch up and to share a few pics from my visit to the HIV ward of Georgetown Public Hospital. Yesterday was my birth anniversary so i felt the urge to take breakfast for the patients and nurses of the HIV ward of the Hospital on Saturday.



All went well as planned, I prayed with them, we talked, laughed and i took the opportunity to empower them on being more proactive upon returning to society.

Good Communication

Clinicians are part of your clinical team. If you are in a peripheral facility, methods need to be developed for good communication and at least yearly meetings.

Communicate with the district hospital/clinic concerning all chronic patients,
even when treatment is initiated at the first level facility.

Coordinate care with appropriate clinic/clinicians.

Refer patients back to clinicians as appropriate.

The power of SALT

Its my privilege to apply the SALT after being ignorant of sum facts. Now, as a team, we will continue to empower prisoners and those in the rehabilitation center so as to produce better

rounded individuals who will be reintegrated into society. When the message goes into the home then we

are better equipped to deal with the community, it will then spread like a
