HealthRise funded by Medtronic Foundation, implemented by Abt Associates is working with partners MAMTA Health Institute for mother and child and Catholic Health Association of India since Nov 2015 across the continuum of care to increase outreach, screening, referral, treatment and care in Udaipur and Shimla for Hypertension and Diabetes. The intervention covers an underserved target population of over 90000 (age15-70) from rural and urban blocks in each district.
In India, HealthRise has developed a decentralized integrated district level model to bridge gaps in implementation of NPCDCS guidelines by strengthening health systems at the primary care level. We have developed a community based patient care model which was integrated with the local Govts. to strengthen primary care by providing training and technical assistance for early diagnosis and improved treatment outcomes for Hypertension and Diabetes
We are planning a webinar on 17th July 2018 from 03.00 p.m to 04.30 p.m to share our results, some key accomplishments, learnings and insights from the work undertaken in expanding access to care and treatment for hypertension and diabetes in Shimla and Udaipur.
