SUMMARY:Non-communicable Diseases Webinar
DESCRIPTION:HealthRise funded by Medtronic Foundation, implemented by 
 Abt Associates is working with partners MAMTA Health Institute for mo
 ther and child and Catholic Health Association of India since Nov 2015
  across the continuum of care to increase outreach, screening, referra
 l, treatment and care in Udaipur and Shimla for Hypertension and Diabe
 tes. The intervention covers an underserved target population of over 
 90000 (age15-70) from rural and urban blocks in each district. \r\n 
 \r\nIn India, HealthRise has developed a decentralized integrated dis
 trict level model to bridge gaps in implementation of NPCDCS   guidel
 ines by strengthening health systems at the primary care level.  We
  have developed a community based patient care model which was integra
 ted with the local Govts. to strengthen primary care by providing tra
 ining and technical assistance for early diagnosis and improved treatm
 ent outcomes for Hypertension and Diabetes  \r\n \r\nWe are planning
  a webinar on 17th July 2018 from 03.00 p.m to 04.30 p.m to share our 
 results, some key accomplishments, learnings and insights from the w
 ork undertaken in expanding access to care and treatment for hypertens
 ion and diabetes in Shimla and Udaipur.  \r\nRequest to join the webi
