Without laboratories to test anti-TB drug resistance, how will we cap MDR-TB/XDR-TB?

today a news item is disturbing:

It says that India doesn't have enough drug susceptibility testing laboratories to test anti-TB drug resistance - so how are clinicians going to diagnose and properly treat MDR-TB/ XDR-TB? Is it not likely that patients of MDR-TB/XDR-TB might be further exposed to drugs that they might be resistant to, and make them resistant to other anti-TB drugs as well?
It will be good to learn from you all on how doctors diagnose/ treat patients with drug-resistant TB in your communities? Are there laboratories in your countries or places you live in?
